The personal information subject to disclosure acquired by C3 Advance Technology Co., Ltd will be used only for the following purposes. In addition, the following personal information is not subject to disclosure regarding personnel evaluation of employees.

Disclosure of personal information

For notification of purpose of use, disclosure, correction, deletion, suspension of use, refusal of use and provision of personal information, please request “Invoice for personal information disclosure” to “Contact Information” stated below, and please mail it attaching required documents and fees.

About identity verification documents

When requesting the disclosure of personal information, please mail one of the following documents to confirm your identity along with the “Invoice for personal information disclosure”. If your permanent address is listed, please blacken it before sending your documents.

1. When you, not an agent make a request

   • Copy of driver’s license, passport, resident list that can identify you

2. When an agent makes a request
   If the agent is going to file, please mail one of the following documents in addition to the “Invoice for personal information disclosure” and (1) above.

   • Copy of agent’s driver’s license, passport, resident’s card, that can be identified by agent
   • If the agent is a lawyer, a document with a registration number
   • Power of Attorney to indicate an agent

About fees

For notification of the purpose of use and disclosure of personal information, a fee of 540 yen (including consumption tax) will be collected for each request. When mailing each invoice to us, please enclose a stamp or postal flat-rate exchange for a fee of 540 yen. If the fee is insufficient, we will let you know. If you have not paid the fee within one week after contacting us, your request will be invalid.
